I had to take the page down b/c it's directing to my personal page and not the checkout page. And if i go back to edit the form - i can no longer find the toolbar with the thank you page integrations etc. Even deleted and started from scratch. That option no longer shows up.

I am not sure if there is any limitation in your PayPal account checkout process. I did test your form and it does shows option to pay by credit / debit card and product details are also displayed. Please check the screenshot below:
I would suggest you to please check your PayPal account and customize PayPal checkout option: https://developer.paypal.com/docs/classic/admin/checkout-settings/#customize-the-checkout-experience
