I created a form and its not showing up under my account

    I had made a comment about this prior because I cannot lose this form and they told me it was caught in a system and would show up eventually. It has yet to do so. 


    Please advise me of how I can get to this. Thank you!

    Sorry for this inconvenience. I am trying to investigate what caused this issue but I can't find the form in our system either.

    Would you be amenable to just cloning the said form to your account?
