Hi, I created a nomination form for an award ceremony using JotForm, we are all delighted with it, but unfortunalty the other day when i went to login and check nominations, i got a wrong password error. Even following the process for forgot passwords, i never recieved an email. I have followed the link to the form we have sent out and it says the form has been disabled by the owner? I am really worried we may have nominations sitting which we are unable to see, i really hope you are able to help.

I checked your account which is related to the form, it appears to be fine.
However, for the form, I see that it was scheduled to be disabled on Oct 1, 2017
If you are having difficulty logging in, you may reset your password by clicking the Forgot password link below the login fields

Thank you so much for checking, unfortunalty i can't seem to log in to change, have done the reset password several times, with no luck.

I noticed that there is a typographical error on the email address of the account. I have fixed it for you and sent another password reset instruction. Please check your inbox or your spam folder for this email.
