I added Mailing Address and changed pricing and it is not showing up with the new changes on the website where I embeded the code several times.

As quoted by our Founder Mr. Aytekin,
"This is a data replication problem between our servers in different data centers. You may either wait little bit for the changes to be updated, or you can use the same domain name. For example if your form is on jotform.us, you can login and use the form builder on www.jotform.us. Then there will not be any lag in the updates. Usually there is no lag but we are doing some database maintenance today."

Addition to my response, it seems that the form you have provide above was autosuspended by our phishing detector.
