How to create a form where ALL the questions and fields are one ONE page only

I am trying to create a form where ALL the questions and fields are one ONE page. Every time I try to create a new form, even using a template or theme that shows this style of form, my form always turns into a "enter this info, then click next" style of form.

I am not sure how else to convey this issue...

I want:
Name: __________________
Address:_________________
City: _________ State: _____ Zip:_____

NOT:
Enter your Name: _______________  >>next

How can I accomplish this?

Your form is using the new Form Layout the reason it has multiple pages. Set it back to Legacy layout Form Layout and it should fix the issue.

Go to form settings click "SHOW MORE OPTIONS" then, scroll down to the last part of the page. Change the "Form Layout" to "Legacy Layout"
