Address Field Missing In PDF

Despite having all the settings right (I think), for some reason, my EMT form will no longer copy mailing address field to PDF when I create a PDF to print.  Help please!

Please try resetting your PDF. You can do that in the PDF Designer page. Here's how:

1. Log in to your account.
2. Select the form and click the Submissions button.

3. Click the PDF and select Customize PDF.

4. Click the Reset button and then click the Save button when you're done.
