We are running a raffle and would like to limit the entry to one per person. We are requiring e-mail, phone number and name. Is there any way to do this?

Yes, that would be possible. You can set up "Unique Question" question on your form to do that.

You can also limit form submissions based on multiple fields in your form.

You will not be able the form twice using the same email address in your first submission.
When you attempt to submit the form for the second time using the same email address, you will get the following warning message:
