Product order form

I created a product order form and I am trying to find a way to choose and submit multiple sizes and types for the products and tally them to the final order. 

for example: customer can choose a women's tshirt in size medium as well as a large.  

Here is how you can access those options in Purchase order integration:

And change the size, type, quantity for each product.
