Locate specific template in the form templates

Trying to locate a template called Premier Designs Style Quiz, created by Deborah Bush Raley.  I am a subscriber to her group Pocket Deb and am allowed to use her template.  Following her directions, there is no "type" I am to fill in.  She says just to search for that template and it will come up.  Not happening for me.
