It says form was deleted?!

I exported the list from Jot form but by no means did I delete it...however that's what it looks like on the backend now.

Please let me know how I can get the form back up! 

I can see your form is available online.

Can't you see your form in your https://www.jotform.com/myforms page? Try clearing your browser cache using the guide found here http://www.refreshyourcache.com/en/home/

Also try checking your form in the "Trash" folder, if the form I viewed was not the one you mean.
