Account autosuspended

When I try to login I receive a message stating my account is autosuspended. Please help.

The form Independent Contractor Agreement (Photographs) was found on your account as suspicious of phishing activity by our automated Phishing detector system, because it asks for login credentials(username/password) on plain text fields

Gathering this type of data in plain text fields will violate our Terms of use. We have removed the guilty fields from the form. Your must refrain from doing a new form like that to prevent a potential suspension of your account.
