Account status shows that we are reaching our limit of 1000 per month. However since beginning of Sept, we have only submitted 50 or so items and in 3 months we have submitted 1200 (avg 400/mo). I'm not sure how to tell how JotForm is counting our submission usage.

Submission quota accumulations are reset every first day of the month.
But for your case, it is an issue caused by data replication delay/problem between servers, so some of the counters where increase incorrectly even though the submission are correct.
Our development team is already aware on this issue and they already migrated our database to a more powerful server cloud.
Please check data on the Excel and compare it to your emails and is see any missing entries in the Excel then please let us know so we can look into it further.
