the autoresponder/notification email wont display all the fields.
When I first set it up it did, then somehow it changes when I publish it and then it won't display all the fields.
I tried 3x to set up new emails. I keep getting the same result.

Apologies for the inconveniences this may have caused to you.
I have been testing this and it seems like the email notification is somehow corrupted, I can see it shows fine and includes all the fields when it's been added the first time, but as soon as you leave the form and open the email again the content will not be properly displayed:
I will report the issue to our second level so our developers can have a look on this, we will let you know as soon as we receive any updates on this.

Please do. This is the second MAJOR glitch with Jotforms that I have experienced. It is costing us $100's of dollars in wasted time, and makes a $5,000 website ineffective. My client is very dissatisfied with Jotform, as am I. If you are going to advertise that you have an effective service, please fix this error. The last problem was a complete deletion of the saved changes and deletion of ALL calculations within the page which had taken over 60 hours of programming to create. Please make this a TOP priority fix because we cannot operate our business without this being operational!

The issue has now been escalated to our developers. Upon checking, the priority level is now under important. However, we cannot provide you an ETA when the issue will be fixed.
