How can I convert a pdf link to a form?

    Our webpage www.yokefellowpa.org on the home page we have our volunteer application.  I would like to use that form on jotform but when I enter the webpage it doesn't work.  I guess because it is in a different format.  Can you help?

    There are no forms on that page http://www.yokefellowpa.org/ - Our clone feature only works if the form is directly embedded on the website. The system will try its best to mimic/copy the fields but the look and feel will be totally different.

    What you have on that page is a link to a PDF file, which appears to be a paper-based form. The best recommendation I can give is for you to just build the form from scratch. This will give you more control and flexibility on designing it to fit your needs.
