My IP address is blocked and I am unable to access my JotForm account.

    I am a U.S. resident, however I am at my home in Costa Rica and cannot access the site. My IP has been blocked and I cannot login. I had been able to login earlier. 

    I ran malware check on my system to make sure everything is okay, and nothing was found.

    I need your help to complete my task!

    Your account (medleytelesis) is currently suspended. Our phishing detector tool automatically suspended your account.

    The form (Membership Registration) is the one triggered the suspension. Please take note that collecting login credentials are not allowed here in JotForm. I suggest you check our terms of use.

    I removed the password field. Your account is now activated.

    You can prevent this from happening by following our terms and conditions. Please check this link: https://www.jotform.com/help/8-Terms-of-Use.

    Normally, an account will be suspended if you have these fields in your form:

    1. Collecting login credentials (Username and Passwords).

    2. Asking for sensitive information like Social Security Number, Driver's License and Bank details. If you need to collect this data, you need to have a Terms and Digital signature.

    3. Asking for credit card information using regular text fields. You need to use a payment processor if you need to collect payments from your users.

    I am still unable to access my account. I did remove my cache and cookies, but I am still getting the screen attached. How much longer will it take to resolve this issue.

    I also checked your account and did not find any issue with that. Your account is active and you should not have any trouble in sign in to your account.

    What is your current IP address from where you are trying to sign in to your account? Please let us know your current IP address and we will whitelist it from here. Please click on the following URL to know your IP address:  http://whatismyipaddress.com/
