I cannot get my self-grading form to work.

I have been working on this for four hours, watching the help videos. 

What have I done wrong?

I have never had so much trouble. I keep thinking it is something really simple. I cannot even seem to recreate the simple calculating forms in your tutorials.

I did check your form and found that you have added several show/hide conditions along with several disabled condition to insert value as well. 

I would suggest you to please explain what is not working in your form and I will take a look. We will also try to help you solve the problem.
