If I host the form on my own website, am I still restricted to the 10K monthly form views?

Forms embedded to web pages are still hosted from our servers.  Each time an embedded form is loaded it counts as a view the same as it does when the form is loaded by itself.

How can I host the form on my website? Is it possible to do that on another
    subscription package?

You can download the full source code of your form: https://www.jotform.com/help/104-How-to-get-the-Full-Source-Code-of-your-Form

And hire a developer to host it in your site, and  forward the data to your own database if you want to.

Sorry if I'm asking the same question again but I just want to confirm the
    terms of use:
If I download the full source code and deploy it in my web application and
point all the data to my own database, I don't have a limit on storage or
number of form views or form submissions in a month?
Thank you for all the information, you were very helpful!

Correct, you need to make sure that it does not longer submit to our database.
