Creating health survey forms

Hello,

I wanted to know if you have a template for a "how healthy are your survey" form for potential clients to fill out.  I am wanting to create like a funnel system with the info I will be generating from the clients.  Based on the response I get if one is needing attention with their health then I could go to the next funnel system.  Please advise...
