I have a form that creates a rather long PDF so I've decided to enable "Auto Hide Empty Fields" to reduce the number of pages.
The issue now is that I want to have headers on each page so it's easy to know what sort of form it belongs to. The headers I was using were just added there and have now shift upwards because of the hidden empty fields.
Is there a workaround or solution to this that you could suggest? That will keep the headers at the top of each page?

Unfortunately, there is no lock for positioning the headers in a certain page. It will automatically move depending on the length occupied by the other fields.
What I would suggest is to check the add-on feature on Google Spreadsheets to see if it's possible on their end to make this happen. With the Google Spreadsheet add-on, you can create PDFs of your form submissions as well if you integrate your form with it.
