Need help in creating a particular type of form

I am looking to take a subscription out with yourselves. But I need to create a particular set of forms.

I have attached an example of something which we would like to replicate

The form seems to be created in a different account and you clone the form to your account in order to replicate the same. You can modify the cloned form in your account as per your requirement. Please refer to the guide below that can help you with cloning a form to your account.
