Our IP has been banned. Why ? We need to get our forms back up ASAP. We're in the middle of a marketing campaign.
Aren't we supposed to receive a notification when something like that happens? Our forms have been down for the whole weekend !!!

I have reviewed your account and found it was suspended due to this form http://www.jotform.ca/form/72085023479256. However, upon reviewing the form I can see it's clean so I have reactivated your account now.

Your account is active now and your IP address whitelisted; however, may be that you're using a different IP from the one save on your JotForm account and this is why you cannot log in (maybe it was blocked before). So please check again and try using incognito mode, if the problem persists, please provide us your current IP address, this tool will help you getting it: https://whatismyipaddress.com/
I've also cleared your forms cache and they should be loading fine now.
Please let us know how it goes.

Now, there a few thing that we would like to know :
1. What caused your system to ban our Ip ? You said that you found that our account was suspended due to this form http://www.jotform.ca/form/72085023479256. We would like to have more details regarding this matter.
2. What steps can be taken so that it will never happen again ?
3. What mechanism are in place to inform us of a break in the system. We only heard about the incident three days later from one of our future students. We cannot allow our forms to go down again in such a fashion. We have multiple forms feeding multiple systems and they must be available 24/7.

1. It seems like your account was suspended by mistake to the form http://www.jotform.ca/form/72085023479256, I have reviewed it and it does not ask for information that violates our terms nor on previous versions of it.
2. Your forms are manually checked if one of them triggers the phishing alert since you've a paid account, your forms will not be suspended as long as you do not ask for information that goes against our terms.
3. For manually reviewed accounts we use to send an email notifying the phishing form found. Although I cannot find any email that was sent to you about this suspension; however, if it happens again we will notify you as soon as we detect the forms so you can correct them.
Please also accept our sincere apologies for the inconveniences this caused to you. If there is anything else that we can help you with, please let us know.
