I thought I was paying for submissions per month...what is the form view that I've exceeded?

What exactly does a form view mean?
Form view is sometimes referred as impression and it is any type of loading the form. So if you have a page on your website that gets 1.000 views per month and embed the form on the same page, you are OK with the free plan - as long as form views go. Each time someone loads your page would load the form and that would be a form view.
If someone goes to edit their submission - that is another example of a form view.
Disabled forms will however not increase your form views.
You can know more about form views from here: https://www.jotform.com/help/355-Form-View-Limits-What-Is-It-All-About
I have created another thread for your second question to keep the things in order.
