Hi, I have gone ahead and added an Image (File) Upload field into your form for you. Tell me if you like what i've done for you and also feel free to edit it, move it, and configure it to wherever or however you want. I hope this helps.

    @macabbi

That might be due to the delay issues we had. To view your updated form, please change "form" to "www" on your form URL. Example:

    FROM:

    http://form.jotform.com/22503388057151

    TO:

    http://www.jotform.com/22503388057151

    ----

Images will be uploaded to JotForm servers. You can integrate your forms with dropbox or to your FTP so you'll get a copy of every images uploaded via your form.

Your form does have form notifications so you'll be able to recieved notifications with their complete info when someone submits the form. You can also add the IP address to your form notification (use {ip}) for additional identification.

Hope this helps. Let us know if you have further questions. Thanks!

1, neither of the link work. 2. Can the image be named the person’s name from the drop down? 3. How do I direct the images to my drop box.

3, Can I change my username to “maccabi”, pass word can remain the same.

    Thanks,
    Jason

    1. Please clear your browsers cache. That might help. Or, open your form using "www" domain and press ctrl+f5 on your keyboard to force a cache refresh with that specific URL.

    2. No, we do not have such feature for auto-renaming files depending on the form fields.

    3. Here's a guide for that: http://www.jotform.com/help/77-Upload-to-Dropbox-with-JotForm

    4. Done. Please logout from your account and log back in using your new username.

    Thanks!

    Hi, just wanna ask.

    Ive already made my form.

Name, Email and Order Number with UPLOAD FILE.

The thing here now is, once someone has an entry, I can see their info but I can not see the photos uploaded.

My files are saved as .PDF

    Any ideas?

@RaidersRugby, we apologize for the confusion. My colleague only tried to explain that you already opened a new thread using your mvaldez username for your new question located here

    http://www.jotform.com/answers/304504

My colleague has replied to that thread. To avoid further confusion, we should discuss further details on your latest thread link above as this is a new inquiry. rather than replying to this old thread of yours.
