Why all my forms have disappeared?

    i had completed 1 form and was almost complete another and they have both disappeared? where are they?

    Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Clone of Industry Request Form" has fields like "Driver License" which makes it suspected of being used for phishing.

    In most States, there is no law that prevents businesses from requesting SSN or driver's licenses, and there are few restrictions on what businesses can do with it.

    However, some states in the USA have imposed restrictions on a business soliciting  SSN's

    Online form builders, including JotForm, have been used for identity theft in the past. We go to great lengths to prevent this. That is why our Terms  will mention the collection of SSNs and driver's licenses to be considered as a Phishing activity.

    Therefore, when you do request SSNs or driver's licenses,  chances are that our automated phishing detector will flag the form(s)  and suspend the account.  If this happens you'll have to contact support to whitelist the form and reactivate the account, explaining the purposes of the form(s).

    However, we recognize that there are businesses that require a SSNs or driver's licenses for legitimate purposes, here are some:

    • Insurance companies

    • Credit card companies, lenders, and any other company receiving a credit application from you

    • The three main credit reporting agencies: TransUnion, Equifax, and Experian

    • Any company that sells products or services that require notification to the IRS, including:

    - Investment advisors

    - Banks

    -Real estate purchases

    -Financial transactions over $10,000, such as automobile purchases; and other financial transactions

    Nevertheless, you must know that since we do store the information in your account, anyone with access to it would also have access to social security numbers. So, in the event of a security breach of your account this could be an issue.

    So, having this in mind, we encourage you to follow this recommendation:

    Add a sort agreement and/or an e-signature field to the form, so your customers will agree to provide their SSNs or driver's licenses along with personal info.

    In conclusion:  If your purpose is not to collect SSNs or driver's licenses from the general public, but from your own customers; if your business is among the list above and if you follow the recommendations; then all this will surely make your forms not to be considered as guilty violating our Terms of Use.

    Hello,
    No we do not fall into any of those companies.
    If we have to meet criteria to have that field on our form, why would you make it available?

    Hi Amy,

    If you really need to collect this information (Driver's License) from the user then please add the following to your form as my colleague Ashwin requested:

    1. An agreement stating the usage of sensitive information. You can use the "Text" tool. You may also use the Short Scrollable Terms widget or Terms & Conditions widget.

    2. Digital signature field. You can use the E-Signature widget or the Smooth Signature widget.

    I also re-activated your account temporarily so you can manage your forms properly.
