How do you create a form table with rows and columns for entering text

    How can I create like the one in the image?

    You can add an input table to the form and then select inputs as a textbox fields. I have checked your form named "Weekly Check Sheet" and seen that you've already managed it as seen below:

    Also, you can embed PDFs or Spreadsheets to the form with our widgets as you can see below:

    You need to enter columns and rows one by one to the input matrix field as you can see in the screenshot below:

    As you can see, the second line of the columns is not possible to enter into the input table.

    As another workaround, you can prepare the table in Google Spreadsheets and then embed it to the form by using our iFrame embed widget.
