I have been suspended from my account

    I forgot my password and after reissuing it, I got suspended from my account.

    Would you be able to give me access to my account again?

    It seems your account has been suspended due to the credit card fields on the form: http://www.jotform.ca/form/71284288716263

    However, I can see the form belongs to another account where you're sub-user account, so you should not have been suspended. 

    I have deleted the fields from the form since asking for such info goes against our terms, please note that even if you're not asking for such info for phishing purposes we need to still act according to our terms

    Your account should be active now.

    I was also suspended from my account and I have no idea why!? Please help!
