Reactivate Account - Sorry I accidentally broke the rules

    Hi Jotform peeps. My account has been suspended, I inadvertently went against the terms of service and asked for login info with one of my forms (my bad, I did not realize this was against the rules). I am happy to remove the questions asking for login details. Can you please help me get access to my account? I am using jotform to help me run my business.
