Your screenshots are not shown here on the form, but I was able to replicate mentioned issue on my cloned form.
It seems that the filed labels from your Autoresponder email do not match with the labels from the Form Builder.
The easiest way to resolve this issue is to delete the existing Autoresponder email and create a new Autoresponder email. I also noticed that the values are not shown in the email Notification as well. You can do the same with the email Notification.
