I embedded the pop up form as a last resort since no other embed methods work anymore and you cannot see any of the dialog. I color of the font is chosen to be black on the white pop up form however its not showing up. Also the terms are set to be diagonal but are showing up vertical instead. I dont remember jotform having so many problems, Ive spent all day on trying to put a form on our website and am about to give it a rest, your form is broke obviously.Page URL:

Can you please give us more details about the problem? What was happened after embedding your form using default embed code? Does it creates code conflict?
Also, have you tried it with iframe embed code? This should work fine. Since form loads in iframe, it wont create any conflict with your page.
I just checked the form on your site and it seems to be working fine here. The pop-up form opens up fine when clicking "Click Here to Buy Username/Password". Just one thing I've noticed is that the form has white background which makes the form fields unseen. Just a little customization with your form and that should be working fine.
