How do I go about reaching you, in order to purge the offending form? Also, which form could it have been? I was unaware of such offense.

    Hi Michelle,

Thanks for reaching out to Jotform Support.  As I have checked, your form M&MET Credit Card Authorization collects credit card information and was flagged as a phishing form.

Please note that the collection of credit card details using regular fields is against our terms of use. Due to our PCI certification, we are required to deny the collection of credit card details on regular forms. But we have other PCI-compliant ways to do this. All you need to do is choose any payment processors available on our platform to integrate them into your forms. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a "Payment Authorization" feature. Kindly follow this guide on How to Enable Payment Authorization for more related information.

To activate your account, we need to delete the offending fields. We just need your confirmation.

Once we hear back from you, we'll be able to move forward with a solution.
