    I tested your form using Google Chrome, but I was not able to replicate the issue you report. The form was submitted successfully and I was redirected to the Thank You page.

    One of my colleagues tested you embedded form on iPhone and he was able to replicate the issue. However, the form is working properly if the direct link is used.

    Please remove the script part of the iFrame code to see if that will resolve the issue. Use only this part of the iFrame code:

    <iframe

          id="JotFormIFrame-73077676375166"

          onDISABLEDload="window.parent.scrollTo(0,0)"

          allowtransparency="true"

          src="https://form.jotform.com/73077676375166"

          frameborder="0"

          style="width: 1px;

          min-width: 100%;

          height:13771px;

          border:none;"

          scrolling="no">

        </iframe>

    Hope this helps to solve the issue.

    Can you try clearing your chrome browser's cache by following the guide on this link http://www.refreshyourcache.com/en/home/ .

    Let us know how it goes. And if you can take a screenshot of the error that would be great.

    The form is working on my end in Google Chrome. Which OS are you using?

    Regarding the iPhone issue - I saw that you did not remove the iFreme part of the embed code like I previously suggested. Please remove the script part of the iFrame embed code and leave only the iFrame part of the code to see if that will resolve your issue. Use only this part of the code:

    <iframe

          id="JotFormIFrame-73077676375166"

          onDISABLEDload="window.parent.scrollTo(0,0)"

          allowtransparency="true"

          src="https://form.jotform.com/73077676375166"

          frameborder="0"

          style="width: 1px;

          min-width: 100%;

          height:13771px;

          border:none;"

          scrolling="no">

    Guys, i am losing business due to this issue with jotform. It has been three days with this problem and no one seems to have an answer. It is taking too long to receive a response on this problem.

    I am not very happy right now. If this issue is not resolved today I will be canceling my pro account with jotform and I will be letting everyone know about this problem. I know many business owners that i've refered to jotform and they will not be happy when they hear about my experience this week. I will also make sure to review jotform accordingly unless this issue is resolved today.

    Sorry for the inconvenience this may caused you.

    My colleague tested your form again on iPhone and he is also not able to submit the form. The form is also stuck on my Samsung Galaxy.

    I am not sure what is causing this issue. The standalone form is working properly.

    Please try to embed your form with the script code to see if that will resolve the issue.

    If the form is still not working, let us know and we will report this issue to our developers.

    I tested the form just now and I can see you have removed the Appointment Widget. It is now working perfectly.

    I have cloned your form and added "Gift Registry" widget which works similar to "Appointment Slots". You may try using that and embed it to your website so we can test and see if it works.
