My form is not working: You can't fill it out beyond the first 2 fields, all of our additional conditions stopped working

    I haven't edited or made any changes to my form but it stopped working today.

    You can't fill it out beyond the first 2 fields, all of our additional conditions stopped working, even though we've made no changes to our form
