Please try using incognito mode on your browser or clearing your forms cache so you load a fresh version of the forms.
Also, I've tested the direct link of your form, are you still experiencing the issue with the direct link?
If the form is embedded, please share us the link to the page so we can further check it.

Please note that asking for such info is not allowed on JotForm, your account can be suspended due to this activity although you're not using your form for phishing purposes.
I would recommend you to check our terms so you know what is not allowed to ask on JotForm.
