We would like to apologize for any inconvenience this had caused you. I have checked your account and I can only see one form that got a high rating from our phishing tool.
I have double-checked the fields in that form and I have not found anything that would be enough to trigger an account suspension. And since that is the case, I have reactivated the account as it appears the fields are clear to me.
You should be able to log in to your account and access your forms again.
