This is happening when Paypal is selected. The cause is that you left the Paypal email blank. You should add your Paypal email to the configuration.

I HAD THE SAME PROBLEM, IT SEEMS TO BE A PROBLEM WITH THE APPOINTMENT SLOT WIDGET. I HAD TO REMOVE IT FROM MY FORM! GOOD LUCK

As far as my test is concerned, it's not. I have selected an item from the appointment slot widget in jenherem's form, and submit button is working. It's the Paypal that's causing it.
