Most often I notice that the associated pdf that is created for the submission has links in it, and I can click the links of missing files and download them, but as you can imagine this is quite tedious. What can explain why the files don't make it into my Dropbox folder in the first place? It doesn't seem to be tied to any one thing - it's quite random.

Have you tried redoing your form's Dropbox integration? If not, please do, then update us on your results.
