Mixed Layout: How can I import an existing form?

When I click on "create form" I do not have an import option.  How do I import a cloned form???

When you click on the create form button you will get a pop up like this one: 

You need to click on the create a form button of any of the layouts, once you do it you will get the following options: 

You will be able to provide the link and import a form.
