Forms shows wrong description when I paste the form link online or in a text message.

the Wrong form name shows up in the email that is sent as well as when I embed it in a text message or post the link somewhere. 

My Form should not be called Shift Vendor Onboard - that was the name of the template I used and the edited/recreated to meet my needs. 

We would like to apologize for any inconvenience. I have tried sending the link to the clone version of your form only in Messenger (which I believe it should just be similar) and it is showing me the correct title/description of the form.
