Clone form but new form is in different format?

Hi - I've cloned a regular form, but now it shows up as separate questions. I don't want it like this. How do I turn this back into a regular form (where all the questions are on a single page?). The form URL is in the field below.

I have cloned a form on your account to test what happens, but it shows as how it is in the original. Can you try again and let us know if the questions are separating as you have mentioned (just in case a bug was existing and could have been fixed)?
