I've checked your form named "OAP Sessions - Registration Form" and cloned on my side in order to test it.
Currently, I do not see any problem with the Ticket Purchase widget as you can see the screenshot below:
When I have cloned the form on my side, the widget has appeared on the form as seen below:
I have clicked on "Update Widget" button in order to be fixed and it is fixed as seen below:
I've also cleared your form caches in order to avoid any possible problem.
Please check your form again and let us know if the issue still persists.
