    Hello,

    Sorry for the inconvenience. 

    When i check mail logs for your email address recreation@cityofcresco.com. Submission notification emails look sent without any problem:

    Can you check your spam/junk folder?

    There is nothing in my spam folder. I did receive test emails when I was
    editing the Notification section. I am using mail.google.com to receive
    emails.

    Hi, I have tested your form and it works fine, I received an email notification.

    It is recommended to set sender address to mailto:noreply@jotform.com

    and whitelist all jotform domains. If you are using custom sender email addresses you can use Mandrill workaround.

    Changing sender address fixed the problem.
