Problems with my Form

    Something happened to my form and know it has numbers where checkboxes used to be and under it also looks like it that way under the billing section. How do I fix it?


    OPTIONAL SERVICES:

    Hello,

    You're having an error because the code in your Multiple Choice Properties>Options

    that you have added is messing up the the checkbox. Delete the code or change it.
