Salesforce integration not submitting all fields

Dear Sir or Madam,

Regarding our form "DACH: Facebook Dentalanalyse":

We installed the Salesforce integration. It is working fine, but data from these fields are not transmitted:

Name:Salutation (First Name and Last Name are transmitted, but not the Salutation Mr/Mrs)

Company (hidden field, by default with the value 'Umfrage')

Lead Source

I think that I added these fields later, so perhaps somehow the integration does not update. 

I deleted the integration and setted it up again, but this didn't work.
