how can i upload my current client forms so that they can fill out on line

Hi . I have a small salon and I have 2 forms for clients to fill out when scheduling a color, chemical, or freelance service. I want them to be able to fill them out prior to arriving for appointment via email or on my I pad. would you system allow for me to so this. I see about 20 clients a month who would fall in this category.

Forms online can be filled up through any browser or a web page (if in case you have a website published). Once an form gets submitted, you shall be notified via email for this. If you have the JotForm app installed on your iPad, you can also get notified on your device.
