My account says it's suspended and I was just charged. Please help me.

We do not allow for the collection of usernames and/or passwords through forms. Your "Website Client Worksheet" form requests both usernames and passwords.

If you would like, we can remove the offending fields from the form and activate your account.

I deleted the fields and activated your account. Your forms should be usable once again.
If any of your other forms request any type of log in or banking information, I would recommend removing those fields to prevent any further issues.
