I want to create the table database

Dear Sir,

I have to create database collection form from my member..
This is off line form View

is there any table option so i can use that widget for adding in jotform..

please help me with solution

You can add the Input Table field to your form. 

After that, you need to open the Properties for more options. Here's a screen capture of my test:
