How can I copy a form?

Yesterday, per my request, you moved a form from my account (username bobspear) to my colleague's account (username csande). The name of the form is "3CS Proposal Form". Perfect. You did exactly what I asked you to do.

Except that's not quite what I wanted you to do. My bad. What I really wanted is to COPY the form; that is, I still want to have the form in my account.

Please help!
