Why is my form not displaying on mobile devices?

    your form on the website page worked ok on Dell laptop

    I have been testing your form and was able to view the form on mobile devices:

    May you please confirm that the form displays now? 

    I have  also tested your form in a similar device than yours and was able to see it working:

    If the problem persists, please try using a different internet connection.
