My link doesn't work

I've been trying to share the link to my feedback form but every time I try to copy and paste it into the search bar or just type it out it says it can't load the page because it can't connect to the server "form.jotform.com"

I have been testing your form and it seems to be working on my end: 

May be that there is something blocking the form so I would recommend you to try using a different browser or internet connection.
