Thank you very much for your input. I made the changes but it is still very slow loading....any other recommendations? Also, I believe this form was built in a shared folder of a sub account ...will I be limited on the submissions I am allowed since I have an upgraded account and the other is free? I will be needing to use this form more than the free 100.

I just tested it, and it loads just fine. Probably the issue is related to your browsers cache, please try clearing your browser cache. You can also try clearing your form's cache: https://www.jotform.com/help/229-How-to-Clear-Your-Form-Cache
In order to transfer the form to your account, the owner of the form has to log in to his account username "jpredmore", then he needs to open a new forum thread to request the transfer. In the request please specify the URL of the form to be transferred, and the username of the account that will receive this form.
